Virtual Talent Show Permission Form

Please fill out this form to participate in Sugartown's Virtual Talent Show (fill out a separate form for each child student you are registering). We ask participants to prepare a 90 second video of their act and submit to us. These talent videos will then be edited into a show to be viewed on February 27th.

 

Important Dates:

  • Talent Show Sign-Up Ends: Friday 2/13
  • Video Submissions Due By: Thursday 2/20
  • Virtual Talent Show Thursday 2/27 at 6:30pm



I give permission for my student(s) to participate in the Sugartown Virtual Talent Show and acknowledge that their submission will be published on a school YouTube video. (reset)

 

 

 


I agree to review my student's submission to ensure all material is appropriate and that the performance adheres to the to 90 second time limit. (reset)
